clarify that QR code metadata is scanner specific in the README
This change - clarifies that not every decoder returns extra metadata. - removes the mention of hexadecomal encoding; some programming languages such as Go and Java return raw byte arrays. - mentions ZBar, a high quality alternative decoder.
