docs: consolidate patch release push changelog

Group the reviewed push transport fixes alongside the authorization and per-ref outcome changes. Describe the final behavior since 3.0.4 without duplicating the sequence of fixes.

This changes only release notes; runtime behavior and release version are unchanged. Validation: reviewed the unreleased entries against the deployed push changes and checked diff whitespace.
