chore: bump @routstr/sdk, ignore npm artifacts, use max_completion_tokens

- Bump version to 0.4.6 and @routstr/sdk to ^0.4.2; declare published
  files (dist, src, README, SECURITY).
- Default request limit now sets max_completion_tokens (OpenAI-standard,
  accepted by reasoning models) instead of legacy max_tokens, keeping
  max_output_tokens for the Responses API.
- Ignore npm lockfiles; scope root index.ts ignore pattern to root only.
