Creating a Marmot group took about a minute. publishAndConfirm waited for
every relay's verdict (15s plus 5s per transport retry) before reporting
whether any accepted, and group creation makes two commits back to back,
so one silent relay in the outbox cost each of them the whole timeout.
Marmot's publish rule only needs one acknowledged accept.

Adds an opt-in untilFirstAccept to publishAndConfirm/publishAndCollectResults
and uses it for the Marmot publisher in the app and amy. The other relays
still get the event from the pool; only the wait is skipped. Every other
caller keeps waiting for all relays.
