Claude b69cb35392 Add per-version changelog files under docs/changelogs ...

Split the root CHANGELOG.md into one file per version (6.1.0 through
6.2.3) inside a new docs/changelogs folder, with a README index linking
each version and pointing to the GitHub releases page for older ones.
